We keep and processes information about you if we instructed by you or other service provider to conduct a blood sample from you for diagnostic purpose. We currently collect and process the following information:
- Name, postal/ visiting address
- Contact information (email address, phone number)
- Demographic information
- Medical history (if applicable)
2. How we use your Information.
Most of the personal information we process is provided to us directly form you or by email, telephone or via our website. We may need to collect information about you form a third party. The third parties from whom we may collect your personal information if they have referred you to us.
These personal information’s are needed to communicate with you and make arrangement for the sample to be collected, and to ensure that the sample is collected correctly. Also, to investigate and resolve a complaint.
If you choose not to provide the information that is asked of you for your sample collection, we may be unable to comply with our obligations to the data controller.
Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for procession this information are: We have a legal obligation: Phlebo will only disclose information about you to third parties if we are legally obliged to do so.

Your information is securely stored. Your personal information provided by your or your service provider will only be kept as long as is required to complete the sample collection and investigate and resolve any complaint within 30 days from using the service. We will then dispose your information by one-site shredding of all your personal data.
